The voltage of the cell are measured via the balance tap at the bottom of the board. Note for 2S #2,3 and 4 are bridged to positive of cell #2, this is the preferred method. I'm not sure how Lost Vape wired theirs. Another way is to bridge batt + with #2.3 and 4, some manufacturers do it this way. I think that you will find your problem with the connection between cell #2 and tap #2 regardless of which way that they wired it. Example would be a poor solder joint, ect.

What does EScribe Device Monitor show for the voltage of each cell?

I will be surprised if it is anything other than what ThatGuySwain posted about. For awhile is was a very common problem. Back then if you had sent your mod to Evolv, he would be the person repairing it.

From here I can only guess but it sounds like a connectivity problem with the 510. Use Atomizer Analyzer to get an idea if this is the case. Note when the connection starts to stop getting lower vs how many turns are left till the atty bottoms out onto socket. Compare the two mods perhaps. The desired situation is the connection is stable about a least a turn before bottom.

A DNA75C with it's up to 100 watt boost (INT firmware) and a 21700 is a pretty capable device. That's a nice one.

That same sled setup would also work well with a printed case. BTW, that's a nice looking box.

You can always save your existing setup under file/save in EScribe. EScribe also automatically saves a back up. You should have no problem removing Ti from any profiles. But to remove Ti completely from your device it needs to be removed form any profiles first. The device can hold 8 materials so unless you need to remove Ti from device to add one I wouldn't worry about removing it from device. Nothing happens till you upload to device, experiment all you want, you can easily put it back if it doesn't meet your expectations. There is nothing in the settings that will permanently harm your device.

Output Gnd and Batt- are internally connected within the board, my opinion is that it will work just fine with or without the Batt negative wire from sled to board. Yes the output ground will need to be attached to the 510 ground.

For the purpose of uploading the theme to this forum just use "Save Theme" on Theme tab page, no need to go into Theme Designer. It should default to the correct file type (ecigtheme), name it what you choose to. Save it to where you can locate it and upload it with your post to this thread. Using Device Monitor with device set on the theme for Replay select cold and live ohms from the tick boxes and fire it. Make sure it is showing an increase in resistance when the coil is heated or Replay won't work. This will establish that your wire is indeed TC capable providing the resistance changes from cold to hot. If there is no change then the wire is Kanthal or Nichrome which will not work for Replay. It looks like from your setting that with a proper wire you should be getting the option to start Replay after firing device.

What is the Cell Soft Cutoff set to?

Have you cleaned the threads of the battery cap and the threads that it screws into on the mod? Does the battery appear tight when the cap is screwed in? Alcohol works well to clean these (91% rubbing alcohol) Is the voltage bouncing with the mod not being fired?

What does EScribe show for cell voltage? Both standing and firing.

This is the version of EScribe, not the version of the firmware on the device. Though is does contain the INT firmware necessary for Replay. ("3. I'm on SP 19 INT firmware")

To check the firmware version of your device open Device Monitor and the version will be listed in the bottom right lower corner. (Example is of DNA75C)

I would think that either EScribe or your version of firmware is not the required international version. Here is what the page with profiles looks like/

Well don't do like I did a couple of weeks ago. I accepted the setting from atomizer analyzer but fogot to upload to device so I had to do it again.

That would be my first choice as well.
